Rework from Unclear Requirements: The Single Biggest Source

A defect introduced at the requirements stage is introduced at the most expensive point in the lifecycle to fix it later. If the acceptance criteria are ambiguous when the ticket is written, that ambiguity propagates through design, implementation, testing, and potentially all the way to production. Each stage the defect passes through adds a layer of cost: the implementation is built to the wrong specification, the tests pass the wrong implementation, the deployment ships the wrong behaviour, and the production incident or customer complaint triggers the rewrite.
Capers Jones' research consistently ranks requirements defects as the highest-cost defect category. IBM's 1-10-100 rule applies with full force: a $1 clarification at the requirements stage prevents $100 of production rework. The prevention investment ROI for spec quality is higher than any other lever in the rework reduction playbook.
Why Requirements Defects Are the Most Expensive
The mechanism is timing. A requirements defect is introduced before any other work begins. It propagates forward through the entire lifecycle until it is caught:

Ambiguous Acceptance Criteria
The story says 'the user can filter results'. Filter by what? Single select or multi-select? What happens when no results match? Does the filter persist between sessions? Each unstated detail is a decision made silently by the developer -- and a potential mismatch with what the product owner imagined.
Fix: Write acceptance criteria in Given/When/Then (Gherkin) format. Force specificity. Every scenario that is unclear is a question to ask before sprint start.
Missing Non-Functional Requirements
The feature is specified functionally but the performance, security, and accessibility requirements are implicit. 'The page should be fast' is not a requirement. '95th percentile page load under 2 seconds' is. Missing NFRs generate rework when the first implementation fails a load test or security audit.
Fix: NFR checklist attached to every story above 5 points: performance target, error handling, accessibility level (WCAG 2.1 AA?), security classification, mobile behaviour.
Stakeholder Disagreement Surfacing Late
Two stakeholders have different mental models of what a feature should do. Neither surfaces the disagreement during spec review. The developer builds one interpretation. Sprint review reveals the conflict. Rework ensues. This is the most preventable failure mode and the most common.
Fix: Three-amigos session (developer + tester + product owner) for every story above 3 points. Explicitly ask: 'Does everyone agree on the expected behaviour for this edge case?'
Gold-Plating
The developer implements more than was specified because it seemed obvious that more was needed, or because it was interesting. The additional scope was not reviewed or tested. The extra code introduces bugs, creates maintenance burden, and often does not match what the product owner wanted.
Fix: Definition of Done: 'Implements only what is specified in acceptance criteria.' Review PR diff against ticket acceptance criteria during code review. Flag scope creep explicitly.
Goal Drift
The requirement is valid when written, but the business goal it serves evolves during the development sprint. The implementation is complete but no longer serves the updated goal. The feature is redone or discarded. Goal drift is particularly common in longer sprints and on features tied to market-sensitive decisions.
Fix: Short sprints (1-2 weeks). Large features broken into thin vertical slices. Explicit 'pivot or proceed' decision at sprint review before the next increment is committed.
Unwritten Assumptions
The specification is technically complete but rests on assumptions that are not written down. 'Users are authenticated' is an obvious assumption to the product owner; less obvious to a developer joining mid-project. 'The data is pre-sorted' is an assumption that, if wrong, changes the implementation entirely.
Fix: Assumption log attached to epic-level specs. Engineers write down assumptions they are making before they build. Product reviews the log before sprint start. Differences become questions.
Prevention Playbook
Spec Templates
Adopt a mandatory ticket template for all stories: User Story (As a [role] I want [action] so that [benefit]), Acceptance Criteria (Given/When/Then for each scenario), Non-Functional Requirements (performance, security, accessibility), Assumptions (listed explicitly), Out of Scope (what this ticket does NOT do). The 'out of scope' field prevents gold-plating and ambiguity about boundaries.
Example Mapping (by Matt Wynne)
A structured workshop format where product, engineering, and QA spend 25 minutes before sprint planning mapping examples for a story. Rules (acceptance criteria), examples (concrete scenarios), and questions (ambiguities) are written on coloured cards. Questions that cannot be answered before the session ends are either resolved before sprint start or the story is moved out of the sprint.
Three-Amigos Sessions
Developer, tester, and product owner review every story above 3 points before sprint planning. The tester's role is to ask 'what about...?' questions that surface edge cases. The developer's role is to surface implementation ambiguities. The product owner resolves them. 30 minutes per story, maximum. Stories that cannot be fully specified in 30 minutes are too large for the sprint.
BDD Feature Files
Acceptance criteria written in Gherkin syntax (Given/When/Then) are executable specifications. When integrated with Playwright or Cypress, the feature file becomes a test that runs in CI. Failing tests = mismatched implementation. This creates an automated feedback loop between specification and implementation.
Spec Review Gates
Any ticket above 8 story points requires a written spec review -- a brief (1-2 page) document covering the problem being solved, the proposed solution, alternatives considered, and risks. Reviewed asynchronously by engineering lead and product manager before the ticket enters the sprint. This is the investment that prevents the most expensive rework.

POOR Acceptance Criteria

- Handle errors appropriately

Every decision is explicit. The developer and tester both know exactly what to build and verify.
